Payment Options

• One-time payment of $250 
• Payment plan $50 deposit and 2 payments of $100 (once a month)
• Payment plan $50 deposit and 4 payments of $50 (every other week)
• Payment plan $50 deposit and 8 payments of $25 (every week) 
• Other - please contact us if other payment arrangements need to be made

Payment Process

After you click on the "Click Here to Make A Payment" below, you'll be taken to Davis Behavioral Health's home page to make your payment.  

Screen 1:

On the Home Page, there is a "Make a Payment" button underneath the "Contact Us" button, which is located on the top right-hand side of the page.   

Screen 2:

Enter the amount of your payment.  

Screen 3:

Fill out the payment form.

Description Field - type MBSR

Customer ID Field - type your first and last name

Click Here to Make A Payment
